Physician holding a full, active, unencumbered license under Ch. 458 (MD) or 459 (DO) â Fla. Stat. § 458.3265(1)(b).
Duties the rule assigns
- Physically practice at that specific registered clinic location (§458.3265(1)(b))
- File quarterly adverse-incident reports with the Board of Medicine (§458.3265)
- Operate the clinic's quality-assurance/incident-monitoring program

Notes
