Applies to 'Organized Ambulatory Care Facilities' (OACFs) — defined as structurally distinct establishments constituted/staffed/equipped to deliver 'ambulatory AND URGENT health care services,' squarely covering urgent care/walk-in clinics. CARVE-OUT: the definition EXCLUDES facilities owned by a professional service corporation or an individual practitioner's own office — a PC-owned urgent care may fall outside this rule; confirm entity structure. Physician/dentist must be 'accessible during hours of operation,' not necessarily on-site.
